We offer patient prescription refills at every office
visit. It is best if you bring a list of your current medications to
ensure timely refills. Should you require a refill outside of a visit to the
office, please give at least a 72-hour advance notice for the request.
To obtain a prescription refill simply call (315) 479-5070
during business hours, 8:30 a.m. to 5:00 p.m. Monday-Friday, and press prompt
number 2. You will speak with our prescription refill receptionist who will
ask:
- Your
name
- Date
of birth
- Address
- Phone
number
- Physician's
name
- Medication
name
- Dose
- Frequency
- Quantity
- Number
of refills you desire
Please indicate the method of processing you prefer, i.e.
fax the prescription to the pharmacy, mail-order pharmacy, mail prescription form to patient,
patient will pick up, etc. In many cases, renewals can be sent to the
pharmacy electronically.
REMINDER: Prescriptions for controlled substances MUST be picked up or mailed. Due to state law, these prescriptions cannot be called in or sent in electronically.
The clinical staff and your physician will review your
request within 8 hours of receipt. Please
be patient if requesting mailing prescriptions as turnaround may be delayed due
to the postal system.
If you or your pharmacy has not received your prescription
within in 48 hours, please call (315) 479-5070 and ask to speak with a nurse
regarding the status.
